One man has died and another has suffered a gunshot wound in a series of mysterious altercations leaving a third man under guard in hospital and a fourth on the run from police.

At about 1.25pm on Wednesday, two men were allegedly shot by another man on a property at Eungella, in the northern rivers region of NSW.

Paramedics treated a 41-year-old man, who is yet to be identified, for a gunshot wound but he died at the scene.

A 38-year-old man, who also suffered a gunshot wound, took himself to hospital some time later.

The alleged shooter, fled the scene with a fourth man, aged 37.

Police were later alerted to another incident on the same road in Eungella where a 45-year-old man had been hit by a car.

He was taken to hospital and remains under police guard while the 37-year-old remains at large.

NSW Police will speak with both the 38-year-old with the gunshot wound and the 45-year-old to try and work out the exact sequence of events leading to the death and multiple injuries.
